The Australian Immunisation Register (AIR) is a national register that records vaccinations given to all people in Australia and includes vaccinations given under the National Immunisation Program, School Immunisation Program and privately such as flu or travel.

How to get immunisation history statements
Parents/Legal Guardians requesting evidence that their child was vaccinated as part of the School Immunisation Program (SIP) can also request this information from the service provider.
Scenic Rim Regional Council is no longer a service provider for the SIP however should you require a copy of immunisation records, you can request immunisation records for yourself or your child by completing and submitting an immunisation history statement request form. Such requests should be made in writing in order to adhere to privacy legislation and submitted via one of the below methods.
Immunisation History Statement Request
Please note there is no fee associated with this request.
The request should include:
- Your full name;
- Your current address; and
- Your contact phone number/s.
In addition to the above, if the request is not for yourself, you will need to include your relationship to the person whose records you are requesting, and the following details of the person detailed within the record:
- Full name;
- Date of birth;
- Current address;
- Address (at time of immunisation);
- School attended (at time of immunisation); and
- Year/grade (at time of immunisation).
